Annual Forms

• Catholic Verification Form
• Medication Administration and Authorization Form — as needed

One-time requirement:

• Present Original Birth Certificate to the registrar, Pam Autry
• From a Health Provider - please submit by Welcome Fair in August:
  • Virginia State School Physical Form - all NEW students and all rising Kindergarten and Seventh Grade students
  • Immunizations - all NEW students, JK students and rising 7th grade students must submit.
