Annual Forms
As we transition to Continual Enrollment for the 26-27 school year, the following forms will be updated for online submission. Should any of the following forms need to be available offline, their links will be included below. Stay tuned!
Annual Forms for the 2025-26 School Year:
- Catholic Verification Form
- Student Registration Form (1 per student)
- Family Data Sheet (1 per household address)
- Health Clinic Form (1 per student)
- Permission to Treat and Assumption of Risk (1 per student)
- PTO Service Hours & Parent-Student Handbook Acknowledgment Form (1 per family)
- External Testing Form (1 per student)
- Transportation Form (1 per family)
- Student and Extended Day Handbook Acknowledgment Form (1 for each family)
One-time requirement:
- Student Demographic Form
- Publicity Release Form
- Present Original Birth Certificate to the registrar, Pam Autry
- From a Health Provider - please submit by Welcome Fair in August
- Virginia State School Physical Form - all NEW students and all rising Kindergarten and Seventh Grade students
- Immunizations - all PK students and rising Seventh Grade
- Medication Administration and Authorization Form - as needed
